We are delighted to be recruiting for an experienced Adult Social Care Practitioner to join a dynamic and rapidly expanding Adult Early Support Team (AEST) within Adult Social Care.

This is an exciting opportunity for a passionate, innovative and motivated professional who wants to make a real difference to the lives of adults in the community.

The Adult Early Support Team takes a strengths-based, preventative approach, providing effective support from the first point of contact. The team works to prevent, reduce or delay the escalation of care and support needs, while promoting independence, resilience and positive outcomes for residents.

About the Role

You will work in a fast-paced, multidisciplinary environment, supporting adults who may be experiencing a range of complex circumstances and social care needs.

Working collaboratively with individuals, families, professionals and community organisations, you will identify strengths, assets and opportunities to support people to remain independent and achieve their desired outcomes.

Key Responsibilities

• Provide strengths-based social care support to adults from the point of first contact.
• Undertake assessments and identify individual needs, strengths, risks and desired outcomes.
• Work proactively to prevent, reduce or delay the escalation of care and support needs.
• Promote independence, choice, control and wellbeing.
• Develop creative and person-centred solutions to meet identified needs.
• Work collaboratively with adults, families, carers and wider support networks.
• Work closely with multidisciplinary partners including social care teams, police, mental health professionals, occupational therapists and voluntary sector organisations.
• Identify appropriate community resources, services and preventative support.
• Contribute to effective safeguarding and risk management.
• Maintain accurate and timely case records and professional documentation.
• Participate in multidisciplinary discussions and contribute to coordinated support planning.
• Work flexibly and creatively in a rapidly developing service environment.
• Contribute to the wider prevention agenda and continuous improvement of the service.

About You

You should have:

• Experience working within a social care or adult social care setting.
• Experience supporting adults with complex needs and challenging circumstances.
• A strong understanding of strengths-based and person-centred approaches.
• Excellent assessment, communication and interpersonal skills.
• The ability to build effective relationships with adults, families and professionals.
• Experience of working effectively within a multidisciplinary environment.
• Strong partnership-working skills and the ability to collaborate with a range of agencies.
• The confidence to work independently and manage competing priorities.
• A flexible and innovative approach to problem-solving.
• A commitment to promoting independence, choice and positive outcomes.
• Relevant qualifications and experience in line with the role's JDPS.

• Relevant qualifications in line with the JDPS
